ABOUT SIS (SHOULDER IMPINGEMENT SYNDROME) AND NONSURGICAL TREATMENT OF ROTATOR CUFF
Hello! My name is Antranik Kosyan. I am a foreign trained Medical Doctor. I graduated from Second Moscow Medical Institute in 1982. Before coming to United States I practiced as a pediatric surgeon for fifteen years. Upon arrival to America I was able to find a job on the medical field. After being in the country for just seven short months, I took a Board Test to become a Surgical Assistant, but I started my career as a Surgical Technician at Presbyterian/St.Luke's Hospital in Denver, Colorado. That was my first introduction to the American Health Care system, more specifically, to surgical services. I met so many great people, that helped me to get accommodated. I did not speak English before moving to United States, so there was, obviously, language barrier at the beginning. But all my coworkers were so nice, polite and very patient. I was accepted to a big professional family of Nurses and Surgical Technologists. I am, still, very grateful for that. My past experience helped me to blend in very quickly. It was so great to feel that the Surgeons, who I worked with, really appreciated my education, experience and skills. After eighteen months being a Surgical Technician I started working as a Surgical Assistant. Three years later I started my own small corporation. For the last twenty years I run a successful solo business as a Surgical Assistant. My passion is DaVinci robotic surgery, that I do in different specialties, but my expertise extends to all other procedures, including General Surgery, Orthopedic Surgery, Urology, Spine and Neurosurgery, Gynecology, Vascular Surgery and Trauma. I do a plastic surgery as well, and it is my contribution in the process of making the world more beautiful.
Shoulder Impingement Syndrome (SIS) is a condition that causes shoulder pain. Shoulder pain caused by SIS is extremely common in the general population. The condition especially affects people who do repetitive activities that involve raising their arms to or above shoulder level, and it is very common among athletes like swimmers, baseball pitchers, football quarterbacks, tennis and golf players and many others. Firefighters and police officers make a different category. The mechanism how people develop SIS is shown in video.
According to different sources and publications, between 250,000 and 275,000 rotator cuff surgeries are performed in United States annually. That is less than 5% of people that have a Rotator Cuff tear. Success rates of conservative treatment are at the range from 62% to over 80%, depending on severity of tear and persistency of the patients in physical therapy and exercise. The best results are in cases when patients do not have tear, but just a weakness of Rotator Cuff muscles that causes an impingement syndrome. These patients do very well and may see improvement as soon as a couple of weeks. From my personal experience, I started feeling better after three weeks.

How did I come up with this idea? I had shoulder problems myself, really bad pain, related to rotator cuff impingement, so I had troubles combing my hair, putting on a jacket or reaching the top shelf, not to mention that I could not play football, or like we call it in the US, soccer, my favorite game. I consulted with the orthopedic surgeons that I work with and asked them for advise. They said that I need to start with physical therapy, so I did. I made an appointment and visited one of CACC Physical Therapy offices, met with the therapist, and he showed me the exercises I needed to do to strengthen the rotator cuff muscles. In order to do that, he gave me a rubber band as the source of resistance. I followed the instructions and after couple of weeks I felt better. In about six to eight weeks I had completely recovered from my condition and started to neglect my exercise. Well, that was a mistake, because my pain came back. I would say, I started feeling the pain three to four months after I stopped to exercise, so I started all over again. It was obvious that physical therapy does work. The only negative thing was inconvenience of exercise. That rubber band provided great resistance, but it needed to be tied up to a door knob or any other strong hook up spot in the house. A door knob did not work well, so I tied it to the treadmill in. Then I started thinking about how to make the exercise easy, convenient and fun to do. I made a few prototypes and, after I saw that it's working, I applied for a patent. Close to the end of the project I met a fine young man, Bobby Johansen, who helped me to finalize the design, created CAD file and found local plastic manufacturing companies that made parts for machine. We assembled the first few units together in my garage. Thank you local companies for willing to take a small, not lucrative, orders and complete them in timely manner. One of the keys for exercising rotator cuff muscles is that upper arm and elbow must be next to the torso in external or internal rotation action. If they are not, you are not doing it right. With the RoC Trainer, the machine controls it, so you don't need to concentrate on your action.
